4.Individualised Patient Treatment

As described above, the three main different pathways (fixed dosing, PRN, treat-and-extend) all have their benefits and problems. The favoured option should be the one that best matches the patient’s individual circumstances. This will depend upon the frequency at which their disease reactivates and the ease with which they can attend the clinic.

From a capacity perspective, it is difficult for the hospital eye service to provide different pathways for different patients since it is easier logistically to adopt one pathway strategy for all patients. However, this results in over-treating some patients and under-treating others, and does not make an attempt to minimise patient visits depending upon their disease activity and their ability to come to the clinic.

The authors’ preferred pathway is to have the flexibility to have the option to offer patients both the treat-and-extend and the PRN pathway depending upon their clinical situation. Although this creates logistical challenges for service provision, it does help to ensure the best care for our patients.
